Foam rolling the IT band feels productive. It doesn't work, and there's a simple reason why: the IT band (a thick, fibrous sheet of connective tissue running from hip to knee) doesn't loosen up under pressure the way a muscle does. You can roll it every day for a year and the pain keeps coming back, because rolling never touches what's actually causing it.
The IT band gets irritated when the hip can't control the leg well enough to keep it from crossing inward with every stride - a motion called hip adduction. That crossing drags the band across the outside of the knee thousands of times over a long run. The band isn't tight. Your hip is weak.
Foam rolling the IT band is genuinely uncomfortable, and that discomfort creates a short-term nervous system response that can dull pain temporarily. It's a distraction technique, not a treatment, and the relief rarely survives your next run.
Hip strengthening - specifically glute medius and glute max work - changes how much your hip drops and rotates during stance phase. Less drop means less friction on the IT band with every stride. This is why runners who add consistent hip work, not stretching or rolling, are the ones who stop dealing with IT band pain for good.
Weeks 1-2: Strength work feels awkward, symptoms mostly unchanged.
Weeks 2-4: Hip drop starts to visibly improve on video; pain during runs starts to lessen.
Weeks 4-6: Most runners report a real, durable shift - the kind that doesn't come back after a long run.
