Lower back pain when running is one of those injuries that runners tend to minimize until it stops them completely.

You might notice it as a dull ache that builds toward the end of a long run. Or a sharp catch on one side when your foot hits the ground. Or stiffness that forces you to stop and walk, stretch it out, then try to run again. Maybe it's been there for months, quietly worsening.

Whatever it looks like, here's what I can tell you after 25+ years working with runners: your back is not the problem. Your back is the alarm. And the source of the fire is almost always in your hips, your pelvis, your core the movement system surrounding the low back that was supposed to be protecting it.


Why Runners Get Low Back Pain

The lumbar spine (lower back) is designed for stability, not motion. It functions best when it's held in a neutral, supported position while the hips and thoracic spine do the moving. During running, your core and hip musculature are supposed to maintain that stability through thousands of steps.

When the hips are weak, the core is underactive, or the movement pattern loads the spine incorrectly the lumbar spine is forced to compensate. It starts doing the work of adjacent joints that aren't pulling their weight, and over time that extra demand creates pain, stiffness, and injury.

Back pain when running is almost always a story of the spine working too hard because something else isn't working hard enough.



The Most Common Root Causes of Back Pain When Running

1. Weak or Inhibited Glutes

Your glutes — particularly the gluteus maximus — are the primary hip extension muscles. During running, they're responsible for driving the leg behind you and generating propulsion with each stride. They also play a critical role in controlling anterior pelvic tilt (the forward tipping of the pelvis).

When the glutes are weak or not firing correctly, two things happen: the hip flexors have to work harder (which pulls the pelvis forward into anterior tilt), and the lower back takes over the propulsion work the glutes were supposed to be doing. Both of those things create low back stress.

The telltale sign: if your lower back tightens up toward the end of a run - when your muscles are fatigued and your form breaks down - glute weakness is almost certainly in the picture.

2. Anterior Pelvic Tilt and Hip Flexor Tightness

Anterior pelvic tilt, where the front of the pelvis tips downward and the low back curves excessively is one of the most common postural patterns I see in runners. It's almost always driven by a combination of tight hip flexors (from sitting) and underactive glutes and abdominals.

When you run in anterior pelvic tilt, the lumbar spine is already in a compressed, extended position before you take a single step. Impact forces traveling up from the ground hit a spine that's already loaded and every mile makes it worse.

This is why hip flexor tightness isn't just a "flexibility problem." It's a spinal loading problem, and it needs to be addressed as one.

3. Inadequate Core Control (Not What You Think)

When runners hear "core," they think crunches and planks. But the core stability that matters for running is different: it's the ability to maintain a neutral lumbopelvic position under the dynamic, asymmetric loads of running.

The muscles responsible for this, the deep stabilizers including the transversus abdominis, multifidus, and pelvic floor, often work well in controlled gym settings and fall apart completely at mile 6 when you're tired and your form degrades. This is when the spine starts taking the beating.

Runners with poor dynamic core control often feel fine for the first portion of a run and develop back pain only as fatigue sets in. If this describes you, core endurance in running-specific positions (not crunch strength) is what you need to develop.

4. Hip Mobility Restriction

Limited hip extension range of motion forces the lumbar spine to compensate. When your hip can't extend fully behind you during the push-off phase of running, the low back hyperextends to create the illusion of hip extension. This pattern (lumbar substitution for hip extension) is incredibly common in runners and is a direct cause of facet joint compression and disc stress in the lower back.

Test yourself: in a standing lunge, can you extend your back hip fully without your low back arching? If the low back arches before the hip reaches full extension, you have limited hip mobility contributing to your back pain.

5. Contralateral Arm Swing and Rotational Load

Running is a rotational activity. Your trunk rotates with each step to counterbalance the opposite leg's movement. When arm swing is asymmetrical, limited, or crossing the body's midline, the rotational forces that should be distributed smoothly through the thoracic spine end up concentrated in the lumbar spine.

Back pain that feels worse on one specific side or that correlates with arm or shoulder issues often has a rotational root cause.


Why Rest Doesn't Fix It

You already know this answer, but here's the biomechanical reason: rest reduces load on the lumbar spine below the threshold that produces pain. The tight hip flexors are still there. The weak glutes are still there. The anterior pelvic tilt is still there. The dysfunctional running mechanics are still there.

When you return to running, you return to the same spinal loading pattern, and the pain comes back (often faster than before) because your fitness has declined and your form degrades sooner.

Back pain when running is a movement problem. Solving it requires changing the movement.


The Corrective Approach to Back Pain in Runners

Restore hip mobility. Getting the hip flexors to lengthen and the hip to extend fully reduces the need for the low back to compensate. This requires more than static stretching. It requires actively training the new range of motion in positions that transfer to running.


Activate and strengthen the glutes. The goal isn't just stronger glutes in isolation. It's glutes that fire correctly during the push-off phase of running, so the lumbar spine isn't recruited for propulsion.

Develop dynamic core stability. Anti-rotation exercises, dead bugs, single-leg loading, and progression toward running-specific positions build the core endurance to maintain spinal neutrality through a full run, not just the first few miles.

Correct pelvic tilt. Retraining the pelvis to sit in neutral, reducing anterior tilt through a combination of hip flexor lengthening, glute activation, and deep abdominal engagement is often the single most effective intervention for chronic low back pain in runners.

Gait analysis. Understanding what's happening at the lumbopelvic level during actual running (not just in the gym) is essential for connecting the corrective work to the activity that's causing the problem.

This is the framework I use with runners dealing with persistent low back pain and why it works when generic "back exercises" don't.



What You Can Do Right Now

Hip extension test. In a kneeling lunge, extend your back hip as far as possible. Does your low back immediately arch before your hip reaches full extension? If yes, hip mobility restriction is contributing to your back pain.

Glute activation check. Lie on your back, feet flat on the floor, and bridge up. Which muscles do you feel working most? If it's your hamstrings or low back (not your glutes) then your glutes aren't firing correctly.

Check your posture on the run. If you have a running buddy or can set up a phone to film yourself, look at your pelvic position mid-run, especially when fatigued. A pronounced forward tilt of the pelvis (butt sticking out, excessive low back curve) is anterior pelvic tilt, the most common postural driver of runner's back pain.

The Bottom Line

Back pain when running is your body's way of telling you that your spine is being asked to do more than its share. The hips aren't moving enough. The core isn't stabilizing enough. The glutes aren't contributing enough. And the back is picking up the slack until it can't anymore.

The fix isn't stronger back muscles or a better mattress. It's correcting the movement pattern that's overloading your spine in the first place.

Runners who do that don't just get out of back pain, they often run more efficiently and faster too, because the energy that was going into compensating for faulty mechanics is now going into forward propulsion.
