The Truth About "Runner's Knee" (Patellofemoral Pain Syndrome) | Ask Doctor Heather

The Truth About "Runner's Knee" (Patellofemoral Pain Syndrome)

Runner's knee gets treated like a knee problem because that's where it hurts. The kneecap is rarely the actual issue - in most cases we see, it's doing exactly what it's designed to do, just getting pulled off its normal track by muscles that aren't holding up their end of the job.

The Muscle Most Runners Never Train

The vastus medialis obliquus, or VMO (the inner quad muscle responsible for keeping the kneecap tracking straight through its groove), is often underactive relative to the rest of the quad. When it can't hold its own, the kneecap gets pulled slightly outward with every bend of theknee. Repeat that thousands of times over a run and you get the grinding, aching pain runners describe under and around the kneecap.

Why General Quad Work Doesn't Always Help

Squats and lunges strengthen the whole quad, but the VMO is often the last part of that muscle group to activate and the first to fatigue. Runners can build quad strength for months and still have patellofemoral pain, because the exercises never specifically targeted the muscle actually causing the problem.

The Fix Is Specific, Not General

Targeted VMO activation, combined with hip strengthening (the same hip work that helps IT band issues also helps here, since hip drop affects knee tracking too), addresses the actual mechanism. Most runners notice a meaningful shift in three to five weeks of consistent work.

Bottom Line

It's not your knee. It's what's controlling your knee, and that's fixable with the right target, not more general strength work.

Train the muscle that's actually the problem.
