Hip Labral Tears in Runners: Symptoms You Shouldn't Ignore | Ask Doctor Heather

Hip Labral Tears in Runners: Symptoms You Shouldn't Ignore

Most hip pain in runners is muscular and responds well to the right loading and mechanics work. A smaller number of cases are something else entirely - a tear in the labrum, the ring of cartilage that lines the hip socket - and running the wrong protocol on it can make things considerably worse. Knowing the difference matters more here than almost anywhere else in running injuries.

Labral Tear vs. Typical Hip Tightness: The Symptoms That Are Different

  • Clicking or catching in the front of the hip, especially getting out of a low chair or pivoting.

  • Deep, pinching pain in the groin rather than a dull ache on the outside of the hip.

  • Pain with prolonged sitting, particularly in a low or reclined position.

  • A sense of instability - like the hip might give way - rather than simple weakness.

Any one of these alone isn't automatically a red flag. Several together, especially clicking combined with deep groin pain, is worth getting imaged before continuing a heavy running load.

Why This Matters More for Runners

Running is a repetitive, high-frequency hip flexion movement. If there's an actual structural tear, continuing to run on it without addressing it can turn a manageable issue into a surgical one. This is one of the few places in running medicine where "run through it and see" is genuinely bad advice.

Bottom Line

If your symptoms match the pattern above, the right next step is imaging and a proper evaluation - not another round of hip mobility drills. Once a labral issue is confirmed or ruled out, we can build the right plan around what's actually going on.

Book a discovery call and we'll help you figure out whether this needs imaging or whether it's something we can address directly through movement work.

Know what you're dealing with. Then treat it correctly.
